Please have your health insurance card, your hospital discharge report and the names of your medical contacts ready for your first appointment .

We meet on an outpatient basis or at your home.

If you are unable to attend, please note that, as with any doctor's visit, appointments must be cancelled at least 24 hours in advance . Emergencies are an exception.
